PERFORMANCE IMPROVEMENT
July 2010 through June 2011


Envision Unlimited looks at outcomes each year to be sure that all of the 1,500 individuals we serve are being supported in the best manner possible. These are some of the outcomes for the year ending in June 2011:

Community Living Services

  • Every new person was admitted to the program in a timely manner (within 13 days)
  • Everyone was provided opportunities for community involvement and integration

Home Based Services

  • 90% of all participants met the individual objectives they set for themselves

Developmental Training Services

  • Every new person was admitted to the program in a timely manner (within 20 days)
  • 68% of all participants met the individual objectives they set for themselves

Respite Services

  • All families were able to receive the additional hours they needed

Employment Services

  • Individuals on average increased their take-home pay this year despite hard economic times

Job Placement Services

  • All employers were satisfied with support services for people they hired
  • Every new person was admitted to the program in a timely manner (within 24 days)

Early Intervention Services

  • Every child  attained the developmental goals set for him or her

Foster Care Services

  • Eight children out of 60 achieved the goal of securing a permanent home this year
  • No child’s foster placement was disrupted

Autism Services

  • 88% of all individuals met the goals set in consultation with them and their families
